Maximum size:  about 25 mm.

Identification:  This is a dusky-gray animal intricately flecked with white and brown. Moderately large tubercles with rust-brown tips cover the dorsal surface. The tubercles are connected by broad ridges and smaller tubercles/ridges appear to replicate that pattern at finer scales. White flecks are concentrated on the ridges while brown flecks are concentrated in the spaces between them. The foot and cephalic tentacles are dusky and lightly flecked with white.

Natural history:  Lamellaria(?) sp. #12 is known from two animals found in Halimeda kanaloana beds at 4-6 m (12- 20 ft).

Distribution:  Maui.

Taxonomic notes:  It was first recorded in Hawaii from Maalaea Bay, Maui by Rebecca Bicker on Oct. 16, 2021.
